All requests must contain:
- Name of student while enrolled at William Daylor High School
- Date of Birth
- Document being requested (official or unofficial transcript)
- How you would like to receive it (pick up, mailed, emailed)
- Contact Information
Student requests must include a copy of their current photo ID.
Requests for Proof of Graduation must include a signed (can be digital) release of information from the student.
All requests require a 1-2 day response time
